*This agreement is valid for a minimum of 12 months.
*Cancellation within the first 12 months of service will result in a cancellation fee for the remainder of the agreement.
*After the first 12 months of service, customer may cancel at any time without a fee.
*Once set up, customer’s bill will automatically draft from the card of their choice once a month beginning the day that the agreement is made and will continue to draft until customer calls to cancel their agreement.
*As long as payments are current, customer’s service agreement will be honored.
*After cancellation, customer forfeits all benefits of program and will need to enter into a new agreement to resume benefits.
